If your son has had a recent concussion, please notify the school. He must have a note from a doctor in order to return to school.

Concussion Information
What is a Concussion? A concussion is a type of traumatic brain injury, or TBI, caused by a bump, blow, or jolt to the head that can change the way your brain normally works. Concussions can also occur from a fall or a blow to the body that causes the head and brain to move quickly back and forth. Health care professionals may describe a concussion as a “mild” brain injury because concussions are usually not life-threatening. Even so, their effects can be serious. New Students
School Health Forms required Per Ohio State law - Failure to meet these requirements will result in the student being excluded from school early in the school year and all days thereafter until the forms are on file. 1. NEW THIS YEAR - All students are required to submit online the Emergency Medical Authorization Form annually. This form, found on Parent Haiku, is to be submitted on line by the first day of school. No other method of submission will be accepted. This one form is used by both athletics and health services. 2. All incoming freshmen and new students are required to have a physical on file in the school clinic by the first day of school. 3. All incoming freshman and new students must have an updated immunization record or letter of immunization exception on file in the school clinic by the first day of school.

School Health Services

A full-time licensed school nurse is provided during the school day to support the health and academic success of students. The school nurse is the link between school, students, parents, health care, and other community providers. Besides coordination of care and being part of the team to support health of students, the role of the school nurse includes addressing major health problems experienced by students. The school nurse provides preventive and screening services, health education and assistance with decision-making about health, and monitors immunization status against preventable diseases. The school nurse provides interventions for acute and chronic illness, injuries and emergencies, communicable diseases, and other adolescent health issues.

Health Clinic

The health clinic is located in Loyola Hall, first door on the left, down the stairs. A pass from the teacher is required to see the nurse. During a student’s free period he is welcome to come to the clinic without a pass. Students are not permitted to stop in the clinic between classes. They must report to their classroom first for a pass. An emergency situation would be the only exception to this rule.

Students are permitted to leave school due to illness or injury only after obtaining permission from the nurse or the Dean of Students office, if the nurse is not available, and with permission from the parent/guardian.

Confidentiality of Individual Health Information

To ensure confidentiality of health information, individual health information will only be shared with certain school faculty and staff for the purposes of serving the student and protecting the health, safety, and learning of this student. Faculty and staff are informed of health concerns on a need to know basis. All staff and faculty who are trusted with personally identifiable health information take seriously their responsibility to safeguard that information.

The school nurse may contact a student's health care provider to clarify facts surrounding a student's condition or treatment plan. Parents are encouraged to talk with the nurse directly if they have additional concerns regarding confidentiality of their student’s health information.
